When clinical documentation lacks specificity or fails to support medical necessity, downstream challenges become much more likely.<\/p>\n<h3 data-section-id=\"7ezr2m\" data-start=\"3223\" data-end=\"3241\">Authorizations<\/h3>\n<p data-start=\"3243\" data-end=\"3425\">Prior authorization requirements continue to grow in complexity. Missing or inaccurate authorizations can create avoidable denials and delays that impact both patients and providers.<\/p>\n<h3 data-section-id=\"14a5n66\" data-start=\"3427\" data-end=\"3449\">Coding and Billing<\/h3>\n<p data-start=\"3451\" data-end=\"3584\">Even small coding inconsistencies can have significant financial consequences, especially when multiplied across thousands of claims.<\/p>\n<h3 data-section-id=\"1rlnv27\" data-start=\"3586\" data-end=\"3618\">Contracts and Payer Behavior<\/h3>\n<p data-start=\"3620\" data-end=\"3776\">Many organizations know what they billed and what they collected. They&#8217;re caused by a lack of visibility.<\/p>\n<p data-start=\"773\" data-end=\"857\">By the time a denial appears, the underlying issue has often been present for weeks.<\/p>\n<p data-start=\"859\" data-end=\"1063\">The organizations that will lead the next generation of revenue cycle management won&#8217;t simply work denials faster.
